About the role:


We have a full time Care Navigator position available in our Care Navigation Team, responsible for covering Gloucestershire Royal Hospital, Cheltenham General Hospital, Community Hospitals across the County and countywide Assessment Bed Units.

You will be working in a team of 12 Care Navigators and supported by two Care Navigator Coordinators. Please, note that weekends availability might be required.


The Care Navigation Team is part of the wider Hospital Discharge & Assessment Team which has a supportive and engaged management structure including an Integrated Social Care Manager, 2 Deputy Social Care Managers, Social Work Leads and Social Care Leads.

It also consists of around 40 other staff members, including qualified Social Workers, Adult Social Care Practitioners, and an administration team.


The role of the Care Navigator is to improve the journey for patients and their carers/family members as they navigate the Health and Social Care system.

You will aim to improve co-ordination and access to support services for patients, with a view to improving their wellbeing, independence, and safety at home.

You will also develop your knowledge of local support services and ensure that patients are aware of support available in their local community.


About you:


As Care Navigators, we understand the work you do on a daily basis and provide the support you need to make a difference to the people of Gloucestershire.


To join us, you must have:

  • GCSE Maths & English Level 94 or equivalent
  • Level 3 Qualification in Health and Social Care, or equivalent Health and Social Care experience
  • Knowledge and understanding of services available to patients/ individuals in Gloucestershire and how to access them.
  • Evidence of good communication skills, good verbal, and written communication.
  • Good IT Skills
  • Ability to work in a fastpaced environment.
