Description
To be responsible for the Assessment and Management of caseload of adult service users who have substantial and complex needs.
Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public.
In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary.
To deliver evidenced based Clinical and Psychologically based Interventions.
The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager.

Newcastle Treatment & Recovery (NTaR) currently has a Band 6 vacancy for an experienced professional to work full time in a service offering support to those experiencing substance misuse difficulties.
The job sits in Newcastle Treatment and Recovery Service based at Plummer Court Newcastle however this post is based in teh community within GP surgeries and includes requirement for home visist.
Due to the logistics of this being based over a wide area, being a car owner is essential to the role.
This post sits within the Shared care Adult Treatment Team and the post holder will deliver clinical based interventions to a caseload of adults who require support to achieve recovery from substance use.
The post holder will have post registration clinical experience in addictions services, excellent interpersonal skills and understand the importance of engagement with not only the client but their care givers and the wider multi-disciplinary team.
