An exciting opportunity has arisen for an experienced registered nurse to join the discharge team based at Frimley Park Hospital.
This will be an exciting opportunity to build on assessment and care planning skills.You will be assisting in complex discharge planning by co-ordinating plans for patients with complex health and social care needs of those whose discharge is delayed, across any clinical specialty at Frimley Park Hospital.
You will have excellent communication skills with a flair for problem solving and the ability to work well under pressure.
You will need to have the ability to work flexibly and independently but also as part of a wide multidisciplinary team.
We are looking for a pro-active, highly motivated individual to work both autonomously and as part of a team. The applicant will be expected to be enthusiastic, organised with excellent communication and IT skills

To promote the continuity of care of patients with complex needs discharged from hospital to community
To provide information, advice and support to health care professionals, patients, relatives and others involved in patient care
To support the MDT to ensure timely and appropriate decisions in the discharge process
To facilitate the planning of complex discharges where sensitivity and persuasive skills are needed
To develop working relationships with Social Services and community staff to enable discharge plans to be put into practice
To gain the knowledge to identify delays in the patient pathway and resolve problems identified
To provide legible and comprehensive patient records to agreed Trust and NMC guidelines
To facilitate a patient focused approach to care that promotes respect and dignity
To ensure that multi-disciplinary staff practice direct patient care in accordance with professional codes high standards of patient care.
Provide support and advice to relatives and carers and ensure effective channels ofcommunication are evident between clinical staff, patients, relatives and carers, being sensitive to the needs of those with sensory impairments or learning disabilities
