Patient support administrator


Salary:
£23,949 - £26,292, depending on experience


This is an opportunity for an enthusiastic and creative individual to shape a role supporting the outreach team for the East & Central Brighton Primary Care Network (PCN).


East and Central Brighton PCN supports patients of 9 GP surgeries generally covering the Whitehawk, Queen's Park, Kemptown, Hanover, Lewes Road and Woodingdean areas of Brighton.


At present, the PCN outreach team consists of pharmacists, social prescribers, occupational therapists, mental health practitioners, health and wellbeing coaches, care coordinators, physiotherapists, nurses and patient engagement workers.

The intention is to grow the team with an administrator in order to support several multi-disciplinary teams.


The PCN team is currently based in an office on Church Street in Brighton and this will be a largely office-based role, though home-working and work in the community are both possible.

This is a full-time position, 37.5 hours per week, on the NHS Agenda for Change salary scale, band 4.

The PCN's principles are:

  • Improve population health with particular reference to the principle of Proportionate Universalism and the need to improve the health of the poorest, fastest.
  • Break down barriers to primary care for patients and community services. In doing so primary care will become more accessible, but will also be more rooted in our local communities and more able to avail ourselves of the range of support services offered to our patients. This is a crucial step towards demedicalising distress caused by poverty, isolation, and social fragmentation.
  • Reduce pressure on primary care to help us deliver on clinical targets, improve the management and quality of life of our patients and provide earlier easier access to physiotherapy, paramedic, mental health and occupational therapy support, as well as other social and community services

JOB PURPOSE
As a PCN we have multiple teams working over 75,000 patients.

This job is for someone who enjoys working with patients, with our clinical teams and has a knowledge of IT skills or a willingness to develop those skills.

In order to help with this we need someone:


  • To coordinate patients with our teams. Maintaining and developing patient administration systems, including appointment systems, referrals and record keeping
  • To improve communication and workflow between patients in the community and our teams
  • To arrange patients to be booked into PCN teams clinics
  • To work within the clinical systems, assisting staff with reports and patients' information and feedback
  • To monitor waiting lists and plan patient activity to minimise waiting times

To undertake person-centred guided conversations with patients, utilising persuasive, motivational, and empathetic skills when communicating.

To support the PCN team in accurate and consistent data recording on clinical systems, and accessing information.


To monitor, report and document any variances in a patient's condition or planned pathway of care to registered members of staff.


To work as part of the PCN team, participating in progress meetings, ensuring that the MDT is kept fully up-to-date with progress reports.

To ensure that all complaints and compliments received are recorded in line with organisational policy.


To recognise the importance of safeguarding procedures within the project, and to ensure that any concerns, disclosures or allegations of abuse are immediately and correctly reported.

To embed equality, diversity and inclusion best practice into all aspects of work.

To work to the requirements of the organisation's quality standards, and other service
- specific quality accreditations as required, abiding by organisational ethos and principles

To attend and participate in regular supervision, appraisals, training and other internal meetings.

At all times provide a caring service and to treat everyone in a courteous and respectful manner.
