Description

Responsible for working with the nurses, doctors and pharmacy to ensure patients receive chemotherapy within specified timeline including co-ordination of test results, scans, prescribing and preparation of chemotherapy prior to admission.

Ensure that consent for chemotherapy is obtained prior to or upon admission to the unit, to support the timely delivery of systemic anti-cancer treatment, with the specific focus of commencing C1 chemotherapy regimens within business hours where reasonably practicable.

Coordinate and expedite the collection of blood sampling and review of results to support timely communication with pharmacy that a patient is safe to proceed with treatment.

Ensure a current weight is documented on nerve centre to ensure the accurate prescribing of systemic anti-cancer treatment. Effectively plan for the elective admissions.

Including, but not limited to the preparation of documentation, liaising with the Clinical Nurse Specialist Teams, coordinating investigations, and effectively communicate with patients and their carers.

In conjunction with the multidisplinary team and service manager, map the existing process for elective chemotherapy admissions and scope systems for improving the visibility of bed management Responsible for identifying potential breaches against Trust targets and taking timely action to avoid breaches.

Receiving triaging and handling calls from patients relating to their admission. To be responsible and accountable for identifying and communicating potential problems at an early stage, liaising with clinical colleagues and non-clinical staff at all levels To be responsible and accountable for cross checking Medway and bookwise for forward planning and accuracy To be responsible for ensuring databases are up to date Deal appropriately with enquiries, by telephone and in person, from staff and patients, seeking advice or referring elsewhere if necessary Working closely with Ward manager to identify appropriate training for staff, to facilitate improved performance and personal career development in accordance with Trust policy/procedures Responsible for management of KPIs to support delivery of Trust Access Targets in line with departmental SOPs To work closely with other members of the multi-disciplinary team to ensure the best possible service is provided
