Job overview:


An exciting opportunity has arisen to join our established team of Advanced Clinical Practitioners in the Hospital at Night service at a time of development and expansion.


The post offers clinicians a wide and varied role working closely alongside all specialty teams out of hours, including neurosciences, acute medicine, respiratory, renal and general surgery.

The post holder will take a lead role in the advanced clinical assessment and treatment of a diverse and undetermined caseload of patients, who may have highly complex and/or chronic presentation.


Main duties of the job:


The post holder will provide expert clinical expertise for developing and enhancing the service and will determine clinical diagnosis and treatments indicated and maintain records as an autonomous practitioner.

This will be done in collaboration with healthcare colleagues.


The Hospital at Night Team consists of Advanced Clinical Practitioners, Site Co-ordinators and a Clinical Support Team who work closely out of hours alongside all specialty teams including the departments of neurosciences, acute medicine, respiratory, renal, general surgery and orthopedics.

Detailed job description and main responsibilities:
The post holder will ensure that evidence-based practice is inherent in all aspects of care and treatment and ensure this knowledge is disseminated throughout the clinical area, Trust and externally as appropriate


They will:

  • Undertake a significant clinical caseload to an advanced professional standard using specialist knowledge from theoretical and practical experience.
  • Develop an extended scope of practice beyond their primary profession.
  • Triage referrals either independently or with medical colleagues and make onward referrals.
  • Initiate/participate in audit projects and work alongside the MDT in diagnostic/prognostic research.
  • Will be accountable for own actions and act in accordance with the NMC Code of Professional Conduct/ Health Care Professionals Council Code of Conduct
  • Provide a professional portfolio of skills including clinical assessment, basic diagnosis, treatments, and advanced practical skills.
  • Prescribe from a locally approved formulary and prescribe within agreed limits under supervision.
  • Perform at a high level of complex decision making in relation to the assessment, planning, intervention, and evaluation of patient care.

  • Excellent clinical reasoning/clinical decisionmaking skills
  • Evidence of involvement in the development of programs of care, protocols and guidance
  • Interpretation of complex diagnostic tests
  • Ability to make effective decisions

Essential criteria:


  • Masters in Advanced Practice
  • Non Medical Prescribing Qualification
  • Extensive post registration experience demonstrating a broad knowledge across a range of specialties
  • Registered health professional with legal and accountable responsibilities for nonmedical prescribing
  • Advanced Life Support Provider
