ocal Cluster Collaborative Lead - City Health ClusterAn exceptional opportunity to lead a Local Cluster Collaborative (LCC) has arisen in City Health Cluster (Swansea).
The LCC Lead will provide senior leadership for the Cluster, which comprises a wide range of partners including the Third Sector, Community Nursing, Mental Health, Allied Health Professionals and primary care services including General Medical, Dental, Pharmaceutical and Ophthalmic services.
Strong and inspiring leadership, bringing the wide range of LCC members together to translate national strategic direction into real action will be very desirable in this high profile role.
The role will report to the Area Clinical Director within the Primary and Community Services, and Therapies Group of the Health Board, and responsibilities include working closely with Health Board Officers and cluster partners to:
- Remodel care pathways
- Deliver services within the community at cluster level
- Support and lead clear plans to improve population health, including playing a key role in steering the direction of the Pan Cluster Plan.
- Helping inform the future direction and maximise opportunities for the role of clusters in the health board region
- Chair Cluster meetings bringing together key stakeholders across the cluster area
- Support the development and delivery of effective clinical governance systems for the Cluster in line with Swansea Bay University Health Board frameworks and policies.
- Lead the Cluster to undertake an annual planning cycle integrated with the Health Board Strategic delivery plans, which is deliverable over three years,
- Identify innovative programmes of work and oversee their successful implementation
- Ensure stewardship of substantial cluster resources, in line with Health Board Standing Financial Instructions.
- Direction and line management of Cluster Multi-Disciplinary staff and Business Implementation Manager
City Health Cluster has a population of over 50,000 registered patients and serves a range of demographics supported by strong multi-disciplinary teams within the cluster.
