Main purposes of the role
To positively support the service delivery team in providing holistic, person-centred support services at Scarborough and District Mencap.


Working to ensure the health and wellbeing of our service users, reviewing Service User files, managing referrals/transitions, crisis support, resolution of queries and concerns in a timely and effective manner.


Working closely with the Assistant Managers and Senior Support Workers to achieve positive outcomes for all our service users across the organisation.

To be the first point of contact for service users, parents and carers.

To provide Duty Management cover in services as needed, to ensure safe services, including evenings and Saturdays.

Lead internal service user meetings with allocated Key Workers and support team.

To ensure care is provided and recorded in line with CQC standards.


Key Tasks and Responsibilities

  • To ensure all Support plans are completed and reviewed in a timely manner, delegating to Key Workers as needed; balancing the need to respond quickly to changes in need with the requirement for a regular, planned reviewing schedule.
  • Ensuring support plans are followed fully and competently by staff, including appropriate activities, communication, use of specific individual aids/equipment, cultural and diversity needs, professional boundaries and relationships etc.
  • Attending multidisciplinary meetings and reviews for service users. Liaising with service users, parent/carers and other professionals, to ensure that service user needs are met, and issues resolved effectively as needed.
  • Manage/assist with allocated service user focused activities, in order to continually review and improve services e.g. consultations, forums, projects and developments.
  • Lead on creation of service rotas, in line with service user support needs, health and safety and continually upskilling the team to support across the services, by scheduling shadowing, competency assessments and development opportunities for staff.
  • Coordinate service user outcomes monitoring and recording of progression of goals, development and benefits of service provision.
  • Assess and investigate issues arising objectively upon the fullest information, such as service user incidents/accidents or support discrepancies in order to review and implement a planned and measured response. Recording all actions taken and resolution.


  • Digital Care records

  • Reviewing and monitoring the online care records in place, to ensure full and accurate records, high standards of care provided and reporting from the system for meetings etc.
  • Leading on any crisis situation arising with a service user/parent/carer, referring to NYCC, other professionals as needed to ensure support in place.
  • Signposting and referrals to professional services for support, including safeguarding referrals.
  • To champion and role model behaviour support practices in services to promote positive outcomes.
  • To offer day to day advice, guidance and support with following support plans, protocols and risk assessments within the daytoday operations of the service.
  • Managing referrals and supporting the transition of new service users into services, ensuring adequate resources in place, staffing etc and providing handovers to staff team.
  • Clear, accurate and timely communication with all colleagues, service users, and professionals.
  • Professional working relationships with colleagues, service users, parent/carers and professionals in line with Management role, organisational policies, procedures and expectations.
  • Completing all allocated quality assurance checks as per the Quality Assurance schedule to ensure high quality support is provided across services.
  • Monitoring and updating service user Health and Safety including risk assessments, protocols, activities etc in line with positive risk taking.
  • Ensure all complaints, concerns and suggestions are documented and reported to the Care and Compliance Manager/ Registered Manager.
  • Plan, record and lead a regular schedule of planned service user meetings, ensuring feedback from the team, that meetings are minuted, actions agreed and completed, and communicated to the team as needed.
  • Positively promote Mencap and our services, attending promotional events and activities, liaising with other care providers, schools/colleges, parent/carer groups etc.
  • Contribute to any surveys or reports required relating to inspections of services, e.g. Executive Committee, CQC, NYCC, CHC.
  • Undertake Parent/Carer Quality Assurance feedback, form link to Parent/Carer Forum and report any issues/comments to Care and Compliance Manager / Registered Manager.
  • Ensure information for Service Users is in Easy Read format/ accessible to individual commun
