Live Well, Health Improvement Services are the creative, passionate and all-round brilliant delivery team for Public Health.

The team's main purpose is to work alongside local people and communities to build connections and opportunities, enhancing and improving lives.

Using evidence
- based approaches and co-creating new ideas with communities and partners, the team strive to create a safe and supportive environment, which enables behaviour change to improve health and wellbeing.


Our projects are multiple and varied, but all are exciting and rewarding and place the person at the very centre.

Projects include (but not limited to) work to reduce health inequality, self-management of long term health conditions, migrant health, discharge from hospital, self-neglect and hoarding, healthy lifestyle change and volunteering.

We have an exciting opportunity for a highly motivated and skilled individual to join our successful team as a Project and Team Lead.

You will be working alongside a committed team of community-based professionals, all with the passion and expertise to deliver a client focused service, helping to improve lives and promote independence.


Your next role


The Project and Team Lead has the opportunity to make a difference during this challenging time, where people are struggling with the cost of living.

Based at Sanford House, our Live Well Hub, this role is customer facing and provides a compassionate and non-judgemental point of contact for individuals seeking information, advice, signposting and support.


Interactions could range from talking about solutions to cost of living challenges, to distributing food vouchers, to supporting someone to navigate the health and social care system.

This role also has strong links with other providers working from Sanford House who rely on Live Well to manage the reception area and work collaboratively to ensure clients receive timely and effective support or information.


Day to day, you will be based at Sanford House which is open to the public between 9am and 5pm, Monday to Friday and the Project and Team Lead will manage the reception area, supporting the triage element of Live Well and overseeing the Household Support Fund discretionary grant.

You will have strong customer service skills with a can-do problem
- solving attitude; always putting customer at the heart of decision making. With exceptional organisational skills and confidence to work with partners in a production and purposeful way. The role is varied and dynamic and so you should be comfortable with ambiguity and thrive in an environment where no two days are the same.

  • You must be fluent in the English language (as a requirement of Part 7 of the Immigration Act "for the effective performance of a customerfacing role")._
