Description

The Physical Health In Mental Health Practitioner (PHMHP) will deliver high level, quality care and evidence-based interventions within a multi-disciplinary team as part of Living Well and the Community Mental Health Framework.


The post holder will provide support and care to service users - most of whom will be SMI (Severe Mental Illness) registered patients receiving care from either or both primary and secondary care services.


The post holder will develop plans with the individual around their physical health and be able to take appropriate action to support them to achieve the planned aims and objectives.


The post holder will be competent in:
Health Promotion

Discussion and signposting with service users around lifestyle, smoking, diet, exercise, diabetes, cholesterol, alcohol, etc. Offer advice on brief interventions and make referrals to relevant services such as weight management or exercise.

Physical Health Monitoring

Able to deliver physical health assessment checks such as the Lester Tool or the SMI Annual Physical Health Check. Undertake Venepuncture as required as well as an ability to undertake ECG as required.

Medical Devices

Undertake quality checks where required. Ensure medical device faults are reported and followed up. Report all problems to Senior Nurse Practitioner.


The post holder will be expected to organise both clinic and home visits with the support of the admin team.


The post holder will be committed to developing and maintaining good working relationships with primary care teams across the area.

The post holder will be expected to review new referrals as part of a wider team discussion, identifying the type of appointment required as well as any potential risks and safety assessing to make a clinical decision on the best type of appointment to offer the service user, be it a clinic-based or home-based appointment

Motivated to maintaining high standards in a changing service

To perform blood tests, ECG, basic observations, smoking cessation and health promotion

Ability to assess capacity to consent and take appropriate action where required

To maintain high levels of infection control


This role requires working across the North East and Derbyshire locality, this includes both the Bolsover and Killamarsh CMHT area.
