We are looking for an experienced Team Manager for the Hospital Integrated Discharge Team.

As Team Manager, you will lead and manage a staff team to ensure we are meeting statutory and locally agreed obligations.

We are looking for an experienced Team Manager for the Hospital Integrated Discharge Team.

As Team Manager, you will lead and manage a staff team to ensure we are meeting statutory and locally agreed obligations.

As Team Manager, you'll also be continually working to improve the efficiency and effectiveness of the Team and of the Department, contributingto business plans and to policy and practice review and development.

The team support the discharge of clients from acute hospital settings and deal with complex situations whilst working collaboratively with other professionals.
