An exciting and new opportunity has risen for 3 full time Band 6 mental health practitioners to work alongside the RAIS team and Fylde and Wyre CAMHS to provide duty cover and crisis cover to A+E and the wards for YP'S under Fylde and Wyre CCG.


This will be carrying out assessments and planning safe discharges or arranging admissions for the young people who are seen.


It will involve liaising and having strong relationships with the local CAMHS team and attending meetings with them to advocate for the YP's.


Signposting YP's to other support will also be integral to the role and a good knowledge of what is available is essential.

It is shift work from 9-12 midnight from mon-fri

Assessment of YP's who attend A+E in a mental health crisis

Carrying out ward reviews of YP's that have been admitted and kept on the ward

Carrying out 7 day follow ups of YP''s discharged from an acute hospital setting

Provide phone support for YP's and families in the Fylde and Wyre area who ring requesting advice and support around mental health

Liaise with F+W CAMHS following each assessment

Attend weekly meetings with Fylde and Wyre CAMHS to discuss new referrals

Work on a shift pattern hours from 9am-12 midnight mon-fri

Help out with Blackpool CCG YP's if this is required

Carrying out comprehensive mental health assessments of YP

Completing safety plans and discharge plans for YP's

Completing reviews after 7 days of being discharged from an acute setting

complete referrals onto to other services

Liaise with F+W so they are aware of all assessments carried out with their YP's

Attend weekly meetings and provide a summary/handover of new referrals and provide professional opinions on which service would be best for this YP

Carrying out ward reviews of YP's that have been admitted and kept on the ward

Provide phone support for YP's and families in the Fylde and Wyre area who ring requesting advice and support around mental health

Work on a shift pattern hours from 9am-12 midnight mon-fri

Help out with Blackpool CCG YP's if this is required
