We are pleased to be advertising this newly created post to work with us in our Community Therapy Team.

This is a new post within an established and dynamic service and you will join the existing team of Associate Practitioners and Techs supporting the registered OTs and Physios.


The post would suit someone who is motivated and has good interpersonal skills, interested in working as part of a team to achieve the best outcome for their patients.

The caseload is very varied, with a particular focus on long term disease management, neurological conditions, rehab, falls and provision of equipment for patients at home, you will have experience in working autonomously as delegated by the OTs and Physios, be able to make your own assessments of patients' risks and needs, and undertake rehab treatment programmes.


You will receive direct line management from a registered therapist who will support you to develop within the role, both in terms of confidence as well as competence, and access further training and development opportunities to help you expand your skills and knowledge as appropriate.


Applicants must be a car driver with business insurance and, although generally rostered to work Monday to Friday, some weekend and evening shifts may be required.

To undertake delegated therapy treatment, rehab and patient care under the direction and supervision of a registered practitioner. To work within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.

To work with patients predominantly in their own homes providing rehabilitation, support and treatment programmes. To deliver and fit suitable functional equipment as prescribed by clinical staff. Supporting clinical staff with treatment and rehab input. To work within scope of competence.

To be proficient, knowledgeable and competent in the delivery of rehab and patient care.

To be educated to an appropriate level and to continue to develop in the role through the acquisition of further skills, knowledge, and competences, to be determined by the needs of patients, the service and the team.

To have a vehicle for use for work purposes and have business insurance.

  • Provide patient focused care, support, education and rehabilitation to patients as delegated
  • Promote patient independence and support patients to achieve their individualised goals
  • To support clinical staff with initial assessment and information gathering for patients
  • Provide a range of clinical support including not only therapy but also basic nursing tasks too
  • Following training, be responsible for a clinical case load as delegated
  • Observe, record and escalate all changes in patients' condition
  • To seek advice if uncertain how to proceed with treatment plans, escalation of patient concerns or in relation to scope of practice
  • Undertake safe and effective moving and handling and positioning skills, often including the use of specialist equipment
  • To support the wider Intermediate Care Team as required, which could include rapid response or support to the rehab wards in exceptional circumstances
  • Demonstrate empathy and effective communication when working with patients, carers, colleagues etc
  • Complete Band 3 core competencies within one year of joining the team
  • Record and report information in a timely and accurate manner, in line with Trust policies
  • Maintain good relationships with all members of the team and others who are involved in patient ca
