Wellbeing Practitioner - Personal Health Budget

Location:
Kenburgh House but working across the District. The role will require travelling to different areas across the district.

Purpose of the post


The purpose of the post is to support the early and safe discharge of clients in hospital, or with a requirement in the community through the use of a personal health budget.

The wellbeing practitioner will support clients face to face.

The primary aim is to enable people, families and carers to access personalised care and support during the process of being discharged from hospitals or with a need in the community.

This will be carried out through the purchasing of goods or services which will enable early discharge of individuals.

Competency in the safe use of bank cards when purchasing goods for clients will be an integral part of the role.

You will be working 1-1 with a small cohort of clients supporting them compassionately to meet their personalised goals within the community.

Main duties and responsibilities

  • Supporting service users from a variety of backgrounds who require support
  • Support clients in the purchasing of goods or service identified need via a personalised care and support planning conversation.
  • Providing support with financerelated tasks which would involve making purchases for clients
  • Maintaining a record of all transactions made including the recording and uploading of all evidence relating to purchases made for clients.
  • To maintain up to date electronic records of support, including client information.
  • Motivate, lead and encourage client participation at all levels to support them to take ownership and manage their own mental health.
  • To make clients aware of Bradford's Crisis offer, Safe Spaces and signpost to other services.
  • Ensure that the rights of people with mental health issues are respected and promoted throughout the organisation in accordance with its values and that clients are treated with respect at all times.
  • Record information electronically and maintain up to date client records.
  • Work effectively with coworkers
  • Motivate and encourage clients to take steps to achieve positive change in their lives
  • Accept and work within the organisations policies and procedures and assist in promoting the organisations aims and objectives.

  • Undertake any other reasonable duties as required to ensure the smooth running of Mind in Bradford.

Organisational and Administration

  • Provide general administrative support to the Finance assistant
  • Ensuring that confidentiality is maintained at all times
  • Be familiar with the Mind in Bradford 'Code of Conduct' and ensure that it is followed at all times both by staff, volunteers and clients.
  • Participate in individual and group supervision and internal/external staff development including mandatory training related to the role.
  • To take responsibility, with colleagues, for ensuring that all Health and Safety, Safeguarding,
Information Gathering and Equality & Diversity requirements are met and all other Company policies complied with.

  • Attend and input to regular team meetings.
  • Ensure understanding of and compliance with all Mind in Bradford policies and procedures.
  • Work in alignment with the aims, objectives, and core values of Mind in Bradford.
  • Perform any other duties from time to time that may be reasonably required to ensure the smooth running of Mind in Bradford.

    Essential Desirable

Qualifications
A minimum of 5 GCSEs (or equivalent) grades A-C - A degree in an area related to must include English and Maths) mental health or equivalent

  • Evidence of ongoing professional development
    A diploma in psychotherapy,:
  • An understanding of safeguarding adults and counselling, coaching or some children and be prepared to undergo training in other professional this interventions
  • Hold an NVQ3 level qualification or above in
Mental Health or similar


Experience
Experience of working with individuals who have Experience of working in or have had mental health challenges. Primary Care settings

  • Experience of delivering 1:1 interventions
    Experience of managing:
  • Confident in managing challenging situations finances on behalf of clients
  • Good IT Skills and experience of using internal recording systems

Skills and
Good, proven communication and First aid certificate
