Description

The Enhanced health in care homes (EHCH) framework is for primary care networks (PCNs) and providers and commissioners of community health and social care services.

It guides the commissioning of and delivery arrangements for health and social care services in collaboration with care homes, so that the entire system works together to provide personalised care, improve outcomes and promote independence for people living in care.

It recognises the life course of people who live in a care home and promotes the ethos of living, ageing and dying well.


Sussex Community Foundation Trust have an important part to play in ensuring this framework is successful and alongside our established Care Home Matrons (CHM) an important and exciting element of EHCH is to introduce telemetry opportunities into the care homes both residential and nursing homes.

We've completed Phase 1 focusing on Nursing Homes and to commence Phase 2 which will focus primarily on Residential Care Homes.

The role would suit a Digital/IT forward positive person comfortable with the use of common IT systems. A knowledge of SystmOne would be useful.

Acts as a key point of contact for the project.

Ensure that Care Homes residents and staff, external stakeholders and colleagues within the Trust receive a courteous and supportive response to a range of queries and requests for information.

Plays a key role in organising and supporting and arranging appointments with the Care Homes.

They will ensure that records are kept up to date and secure and respond to queries and requests for advice from the administrative support team.

Proving administrational support to the other members of the team.

Make initial contact with care homes within Sussex who've shown interest in the Telemetry option. Have sufficient understanding of Telemetry to engage with the Care Home. Training will be given.


Make appointments with the Care Homes to enable the Telehealth in Care Homes Co-Ordinator (TCC) and Telehealth in Care Homes Lead Nurse (LN) to engage with the Care Homes at pace.


Organise and maintain accurate records using a Digital platform such as Excel to ensure the Trust assets are well maintained and accounted for.

This will include any equipment and Digital/IT items utilised by the team.


A calm and approachable manner is essential, along with an empathic understanding that not all of the Care Homes have confidence with utilising Digital/IT systems.

Understanding and maintaining confidentiality at all times is essential and followed in line with SCFT Policies and Procedures.
