    Key Responsibilities:

    • Support the client with functional daily living activities and organisation.
    • Help the client plan and schedule tasks, record information, and complete goals.
    • Provide thinking supports, including linking information together to make sound decisions and solve problems.
    • Collaborate with a case manager and multi-disciplinary team to address physical, cognitive, and linguistic processing difficulties.

    Requirements:

    • Previous experience in a similar role, preferably in complex care.
    • Excellent communication and interpersonal skills.
    • Compassionate, patient, and empathetic nature.
    • Ability to work independently and as part of a team.
    • Flexibility to work specific hours.
