Description
John Van Geest Ward is a 22 bedded ward specialising in acute care for the older person.

This role has 2 key areas of responsibility. You will be responsible for ensuring patient flow throughout the ward is maximised and facilitating the discharge process.


The post holder will provide a comprehensive service to the Trust with a focus to facilitate effective and timely discharge of patients no longer requiring acute care.

The post holder will provide a single point of contact for the management of bed availability and allocation within ward.

The role requires a consistent and coordinated approach to the operational management of patient flow, with an emphasis on maintaining an on-going and 'real time' understanding of the ward's current and predicted capacity position and the timely and well planned discharge process.


  • To support the ward manager and ward coordinator/NIC in maintaining patient flow.
  • To support the ward manger and coordinator/NIC in ensuring all patients have a plan for discharge.
  • To manage the maintenance of etrack to ensure it reflects the current discharge plans and the ward bed availability
  • Liaise with the duty manager when a bed is available.
