See attached Job Description & Person Specification for full details: The post holder will be part of the South Cotswolds Frailty Service; the service supports people living with frailty in the South Cotswolds.

The post holder will work in collaboration with the GP Practice, Community Matron and external providers to create a personalised and coordinated care plan for identified people with frailty.

The post holder will be responsible for developing excellent working knowledge of support available from partners in health, social care and the voluntary sector to encourage and guide this group of individuals to access these services where appropriate.

With the support of the Community Matron, the Wellbeing Coordinator will be responsible for supporting people with frailty in the South Cotswolds by maximising the persons potential for independence and working with health, social care and third sector organisations.

The Wellbeing Coordinator will take a case management / advisory approach for the identified cohort of patients with frailty and will act as a single point of contact for the people whose cases they are managing.

They will develop relationships and an understanding of the people on the caseload that will enable them to spot a decline in health or warning signs sooner and more readily.
