The Role


Our supported housing team, operate three shared HMO properties in Stockton-On-Tees providing accommodation with support to those looking to achieve an abstinence-based lifestyle.

You will be supporting males only who have experienced homelessness or threatened with homelessness.

You will offer a wide range of interventions to help prevent future homelessness by giving tenancy-based support

Once ready for independant living you will help the individual to find affordable move on accommodation

Key responsibilities


You will be responsible to provide daily living support and skills to males aged 18 and over many are dealing with mental health issues emotional needs past trauma and overall complex needs.

Your role will be to support the individual with daily tasks appointments helping the person to live independantly and develop independent living skills You will also be responsible for ensuring that the building is monitored thus providing a safe and secure environment.

This will be evidenced by completing care plans, risk assessments, HMO management jobs and various other duties.


your role will include:
- registering the client with doctors /dentists health proffessionals

  • Work alongside the client to develop and implement a support plan
  • Encourage clients to complete actions within their support plan and following this up to ensure that goals are reached with 4 weekly written reviews
  • Induct referrals into the project, including housing benefit claims, universal credit updates and maintaining an inclusive approach.
  • Prepare accommodation for clients / provide a warm welcoming environment.
  • Liaise and support external agencies such as police, social workers probation officers health proffessionals.
  • Facilitate support in areas such as domestic skills, life skills, money budgeting, personal hygiene, benefit awareness etc. Activities cooking skills
*Ensure all records are maintained in line with policies and procedures

  • Be able to Respond to emergencies and prioritise workload.
*Work in a manner that reflects to boundaries of the role.

  • Complete health and safety and maintenance check which are required weekly such as fire alarm testing, full cleaning, HMO management jobs, completing full health and safety checks and reporting anything to management.
  • Complete full daily room checks, ensuring all healthy and safety is adhered to and that the room is free from drugs, alcohol and visitors.
  • Discussing with the client things that can affect the future tenancy such as tenancy agreements, taking tenancy action anti social behaviour and rent arrears.
*Providing a clean, safe and secure shared space where clients can meet with staff and other professionals.


Team working:


  • To work in a supportive collaborative manner with colleagues and other professionals.
  • Ensure that relevent information is communicated to managers and other agencies as needed but ensuring data protection is followed.
*Ensure the welfare of the client is at the forefront of the support delivered and that risks are identified.

  • Facilitate engagement with the organisation's other projects. To contribute to the effective monitoring and evaluation of the service and evidence outcomes.
  • To work within the organisations policy and procedures.
Training

£10.42-£11.10 per hour
