Professional The post holder will have excellent communication and relationship skills for communicating detailed information with patients and relatives. The post holder will have highly developed physical skills for keyboard and for the use of some clinical tools/equipment.

Contribute to the holistic needs assessment and the development of individual care plans under the direction of the registered nurse Implement, monitor and review aspects of the care plan with the patient and carer, reporting any changes in the patients condition to the registered nurse To triage incoming calls into the service and provide initial triage to patients or carers contacting the service.

Take queries and assess the most suitable method of solution, liaising and discussing complex cases with the Gynaecology Cancer Specialist Nurse Provide general telephone advice, signposting patients/carers to other sources of support as appropriate, such as other Healthcare Professionals, websites, written information etc.

Coordinate appointments or investigations for efficiency and patient experience under direction of the Registered Nurse.

Liaise with the MDT coordinator to ensure a seamless service for patients and smooth transition through the cancer pathway To identify patient/carer general informational needs, accessing / signposting to the Macmillan Cancer Information & Support Centre Identify indication of need or changes in need through telephone contact and respond appropriately Support the registered nurse in the coordination of care to facilitate safe and effective transition between services in order to provide seamless support for people Support the delivery of patient and carer advice and education.

Advise patients on individual self-care management support and education as necessary to non-complex patients, including how to self-assess Encourage and support healthy life style choices Make scheduled catch up calls to patients to check on health and wellbeing and any emerging / ongoing needs and refer to Nurse Specialists if appropriate.

Service development Contribute to the development of the nursing service Comment on proposed changes to policies and procedures to enhance the nursing service To support the registered nurse in the development / management of Gynaecology Cancer Support group / follow up / surveillance support holistic needs assessments and care planning under the direction of the registered nurse To contribute to the Annual Report, collecting relevant data General duties To be a flexible member of the team able to respond to the changing needs of patient / carers To provide a vital link between patients and the registered nurse, highlighting patients that require specialist assessment.


To attend the Gynaecology Cancer MDT Provide support in the Gynae oncology clinics To review patients on in the outpatient settings, providing basic counselling and support as necessary To support the registered nurse in responding to alerts for patients admitted as an emergency To order equipment and supplies To achieve the agreed competencies to fulfil the role with 6 months of being in post Demonstrate an awareness of the limits of own practice and knowledge and when to seek appropriate support/advice To carry out administrative duties required by the role The post holder has responsibility for implementing plans of care with patients and may contribute to modification of care plan.

The post holder will provide advice to patients, relatives or healthcare professionals within their area of designated care.

Human resources To participate in the formal review process and plan future personal development and competencies to be achieved Information resources To record activity on the Somerset database To utilise clinical systems to support decision making Research & Development To support and contribute to clinical audit and research initiatives.

Clinical Governance/Quality/Audit The post holder will contribute to the development of the service, protocols and policies.

The post holder has responsibility for maintaining their own resources in relation to stock control. The post holder has responsibility for updating patient records, data entry, and data processing. The post holder may participate in clinical and non-clinical audits within area of care
