An exciting opportunity has arisen for a Band 5 registered nurse to join the Intensive Home-Based Treatment Team, working with clients within a community setting.

This is a rare opportunity to join a dynamic team


The post holder will be responsible and accountable for the assessment, planning, implementation and evaluation of skilled interventions to address identified needs and risk for allocated clients on a defined caseload.

Works within a multi-disciplinary team as a member of the Intensive Home Based Treatment Team

Staff will receive regular supervision in line with trust policy, and will receive a yearly appraisal. Staff will also undertake mandatory training to be kept up to date to meet demands of the role, and participate within team away days

Staff will have the opportunity to contribute to service development within the clinical governance structure and policies.

Staff are to maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
