Description

An exciting opportunity has arisen for a dedicated and motivated BACPR (Level 4) qualified Cardiac Rehabilitation Exercise Physiologist educated at degree level to work within the Cardiac and Respiratory Services of Birmingham Community Healthcare NHS Foundation Trust Adult Community Services Division.

This is a role within our Community Respiratory and Cardiac teams delivering structured evidenced based exercise and education to service users with chronic cardiac and/or respiratory conditions.


Alongside existing BACPR (Level 4) training, additional training will be provided to meet the service requirements and ensure you are fully supported in your career development.


You will be required to work multi professionally as part of the Cardiac and Respiratory Community Services, supporting patients within their own homes, community clinics and rehabilitation venues across the city.


As part of the team, you will be required to advise individuals with Coronary Heart Disease and/or Respiratory disease on the most suitable form of physical activity.

You will assess, plan and design a comprehensive Rehabilitation Programme that is tailored to meet the individual needs of each patient.

During each programme, your will monitor, evaluate and record patient outcomes, offering advice and support.

To provide a comprehensive city-wide Cardiac and Pulmonary Rehabilitation (PR) service for patients referred to the service across Birmingham.

The service is for patients aged 18 years and above and will be delivered in a variety of locations across Birmingham.


The post holder will report directly to the Cardiac / Respiratory Service Lead Team Leader supporting and assisting them with caseload management.


The post holder will be an active team participant within the service, provide an in-depth clinical knowledge, participate in service improvements to ensure clinical quality is maintained and will mentor, supervise and support the development of junior staff and students on placement within the service.


Prioritise daily work load and work autonomously to successfully manage exercise programme waiting times The post holder will participate in delivering an evidence based education programme to patients, families and carers.

Main Duties


  • To assess, review, monitor and support patients with cardiac and respiratory conditions according to evidence-based guidelines British Association for Cardiovascular Prevention and Rehabilitation (BACPR) and Exercise Training for Chronic Respiratory Disease Level 4.
  • Develop and manage protocols in collaboration with the wider cardiac / respiratory rehabilitation team to ensure that all patients are appropriately assessed with respect to their exercise capacity and referral pathways are defined for those requiring a more detailed review.
  • Ensure all patients baseline observations are completed and recorded on initial assessment and review appointments.
  • Ensure all patients are assessed and given an appropriate individualised prescription for exercise to be carried out either at home or in a community based leisure centre.
  • Provide clinical expertise and advice offering support to design and prepare an exercise rehabilitation programme with the patients consent, implementing and record outcomes throughout the regime.
  • Develop and maintain systems to monitor and evaluate service delivery ensuring evidence based practice.
  • Support patients to maximise their full potential by encouraging them to have a better understanding of their condition, therefore allowing them to work towards self - management as an 'expert patient.'
