Our Mental Health Homeless Team plays a crucial role in delivering specialized care to individuals facing homelessness, ensuring they receive the support they need to improve their mental well-being.


Position:
Band 7 Mental Health Practitioner


Location:
Greater Manchester


Responsibilities:

Conduct comprehensive mental health assessments for homeless individuals
Develop and implement individualized care plans, tailored to each clients specific needs
Deliver evidence-based therapeutic interventions to address mental health concerns
Collaborate with multidisciplinary teams and external agencies to coordinate integrated care
Assist service users in accessing appropriate housing, social services, and community resources
Monitor and evaluate treatment progress, making necessary adjustments as required


Requirements:

Registered Mental Health Nurse or Mental Health Social Worker with relevant professional qualifications
Substantial experience working in mental health services, particularly with homeless populations
Deep understanding of mental health issues and their impact on vulnerable individuals
Excellent communication and interpersonal skills
Ability to work effectively within a multidisciplinary team
Valid drivers license and willingness to travel within the Greater Manchester area
