Band 4 Community Support Workers - 1 Post full time 37.5hrs per week

Great opportunities exist with a vacancies arising for Community Support Workers in the Wandsworth Recovery Support Team.

Successful applicants will have scope to develop as professionals in the field of Mental Health, in a relevantly small multi-disciplinary team.

This Team works with adults predominately aged 18-75 who have severe and enduring mental health conditions in the Borough of Richmond.

The successful applicants will assist the qualified clinicians in the team working as care coordinators, psychiatrists and psychologists to:

  • Provide clearly defined recovery focused interventions, in line with patients' care plans.
  • Actively participate in the Team's meetings including Zoning, seeking to provide highquality services strongly focused on recovery to patients
  • Positively promote and support independent living for patients in the community by maintaining extensive knowledge and links with the community resources and actively supporting patients to access them.
  • Enable patients acquire daily living skills: "living ordinary lives" (e.g. shopping and cooking).
  • Help patients gain access to resources and information about welfare benefits.
  • Help patients access physical health services and gain skills to monitor their health needs in the community on an ongoing basis.
  • Help identify early signs of relapse by monitoring the patient's progress, level of functioning and mental state, updating qualified staff involved in the care of the patient.
  • Support patients develop selfmanagement tools and coping strategies.
  • Maintain adequate records as required by existing procedures; entering appropriate details in case records.
