Description

The Cancer Support Worker will work closely with the cancer dermatology team ensuring the effective delivery of the living with and beyond cancer programme at Wye Valley NHS Trust.

Specifically the post involves supporting clinical teams to identify patients following a cancer diagnosis to be offered a Holistic Needs Assessment (HNA) and provided support with their Health & Wellbeing, both face to face, via telephone or in group sessions.

They will also assist the LW&BC team to co-ordinate and collect data relevant to treatment summaries and holistic needs assessments (HNA).


  • Identify all patients who have recently received a cancer diagnosis and offer a Holistic Needs Assessment.
  • Able to utilise an electronic Holistic Needs Assessment (HNA) to investigate the patient's holistic needs and assist care planning / signposting accordingly.
  • Provide Health & Wellbeing education and guidance to help to empower patients.
  • Signpost and refer patients to suitable services both internal and external to help address patient's holistic concerns.
  • Contribute in the delivery of the HOPE course (Help to Overcome Problems Effectively), designed to support to help patients who have had/ or have cancer to get on with their lives.
  • Given advice about how to minimise the risk of developing further cancerrelated problems.
  • Support the teams in the tracking of patients to ensure all patients receive a treatment summary following cancer treatment
  • Assured confidentiality and discretion at all times when discussing their personal details and needs. This also relates to those close to the patient.
  • Given advice about possible signs of recurrence or longterm effects of treatment.
  • Enabled to reaccess specialist services without delay should they need to do so.
  • Support with the delivery and development of the LW&BC program.
