Description

The purpose of this job is to support the safe management of patients on personalised stratified follow-up pathways following treatment for cancer.

This will be achieved by utilising the Somerset Remote Monitoring System to track patients results, triage calls to the helpline, booking appointments and signpost patients to local and national support services available.


The post holder will also be actively encouraged to identify issues within the cancer pathways they cover and consider changes that will improve patient experience and performance against the national and local targets.


This is an exciting opportunity to be part of a passionate team within Cancer Services who are motivated to improve the care that our patients receive: working alongside other clinical services to improve clinical pathways, find innovative solutions and provide personalised care to patients.


  • To monitor patients on personalised stratified followup pathways, escalating alerts as required to Clinical Nurse Specialist and/or consultant.
  • Utilise the Somerset Remote Monitoring System (RMS) to track patients on PSFU pathway
  • Ensure that monitoring investigations are completed in a timely manner and followup outstanding investigations with patient and relevant department.
  • Organise weekly work lists for review by consultant/Clinical Nurse Specialists
