An exciting opportunity has arisen for an individual to join our expanding Greater Manchester Reconnect service, in the new role of Senior Practitioner.
The post holder will have excellent communication and person skills and have extensive experience of working with individuals in mental health, learning disability or criminal justice services.
Our Greater Manchester ReConnect Service is commissioned by NHS England and is a care after custody service that seeks to improve the care of people leaving prison by offering our service and support pre-release, and peer led navigation and support in the community post release.
The Greater Manchester ReConnect Service is currently provided in HMP Forest Bank, though is expanding to deliver the service in all 5 Greater Manchester Prisons.
The post holder will work within a multi-disciplinary team responsible for supporting individuals leaving the prison setting to enhance their health, wellbeing and quality of life, assisting them to connect with and access health services, voluntary sector services and community-based activities and support.
The post holder will work agilely across the GM footprint to:
- Provide supervision, support and advice to junior staff in the team, in line with relevant clinical pathways as outlined within the operational procedures.
- Assessment
- Formulation
- Signposting
- Close working with all colleagues
- To support with complex cases
