This 'Outstanding' provider rated by CQC is now looking for a Care Coordinator to join their team in their Kensington and Chelsea office on a full time permanent basis.

The role will be planning the work of the carers and ensure that customers are supported in their own homes.


Responsibilities as a Care Coordinator


Ensure the assignment of care professionals to clients, ensuring prompt, safe, and personalised care adhering to clients' preferences as outlined in their care plans.

Ensuring clients receive top-quality care and support, fostering safety and comfort within their homes.

A composed, supportive approach coupled with exceptional organisational and communication skills are indispensable for this pivotal role within their organisation.

Generate requisite reports as directed by the Registered Manager/Directors. Utilise IT systems to allocate suitable care professionals to cater to clients' needs. Collaborate with supervisors to align professionals with clients, considering preferences, travel logistics, and evolving care needs. Prioritise effectively during unforeseen emergencies while promoting client well-being, allowing them to lead fulfilling lives. Active participation in the out-of-hours emergency on-call rota once every 7 weeks in which an additional £300 per week is paid


Your Experience and Suitability for the Care Coordinator Role
Prior experience with Care Coordinator systems is advantageous, but not mandatory. The focus is on individuals naturally inclined towards compassion, care, and patience. They seek a team that genuinely cares for each client as if they were family.

Rest assured, adequate training and development will be provided to ensure you are well-equipped for the responsibilities.

  • Excellent organisational and planning skills
  • Superb communication skills
  • Right to work in the UK
  • Excellent proficiency in English

This full-time role is based in our client's Kensington and Chelsea office, with standard working hours from 8.30 am to 5.00 pm, occasionally requiring overtime.
