Job Summary
To support the Practice with various cohorts of patients to ensure that their needs are met.


Key Responsibilities - Learning Disability champion

  • To work closely with GPs and other Primary Care professionals within the PCN to identify and manage a caseload of patients with a learning disability.
  • Work with patients and their carers to organise and prepare for Annual Health Checks (AHC), enabling them to be actively involved in managing their care and supported to make choices that are right for them.
  • Help to connect patients and their carers to relevant services.
  • Review attendance to AHC appointments and follow up those who have not attended and to reschedule.
  • To undertake AHC appointments in the Learning Disability Care Homes if appropriate
  • Manage weekly contact with these homes to undertake the administrative part of the weekly ward round.
  • Bring together a patient's identified care and support needs and support them to explore their options with the clinicians to produce a personalised care and support plan
  • Health Action Plan (HAP).
  • Ensure that patients and their carers have good quality, accessible information to help them make choices about their care.
  • Promote and enable access to screening and immunisation programmes.
  • Identify unpaid carers and help them access services to support them coding them if they are a patient.
  • Arrange if required and participate in multidisciplinary team meetings when required.

  • Work with the care home, patient and their families in the management their care.
  • Manage weekly contact with the homes to undertake the administrative part of the weekly ward round.
  • Arrange if required and participate in multidisciplinary team meetings when required.

Key Responsibilities - Housebound Champion

  • To work closely with GPs and other Primary Care professionals within the PCN to identify and manage a caseload of patients.
  • Help to connect patients and their carers to relevant services.
  • Ensure that patients and their carers have good quality, accessible information to help them make choices about their care.
  • Promote and enable access to screening and immunisation programmes.
  • Identify unpaid carers and help them access services to support them coding them if they are a patient.
  • Arrange if required and participate in multidisciplinary team meetings when required.
  • Ensure that any QOF reviews are arranged liaising with community teams.

Working Environment

  • Carry out routine checks to ensure your work station is clean and tidy at the start and end of your shift.
  • Ensure all equipment in your work station is operational and up to date and report any issues to your line manager.
  • Ensure that confidential patient information is stored and handled according to protocol and report any concerns to your line manager.

Health & Safety

  • Assist in promoting and maintaining your own and others' health, safety and security as defined in the Practice Health and Safety policy. Ensure that any hazards in your working area are reported immediately to your line manager.

Confidentiality

  • Ensure patient and Practice information is kept confidential and secure at all times, in accordance with the Information Security and Confidentiality protocols.

Performance/Professional Development

  • To participate in training sessions to ensure administrative and computer skills are kept up to date with current technology and practice
  • To participate in regular appraisal and review meetings.
  • To participate in regular team meetings.
