Responsible to:
Surrey Heath Community Providers Ltd (SHCPL, 'The Federation') and Surrey Heath Primary Care Network ('The PCN')


Base:
Surrey Heath Community Providers Ltd, Quatro House, Lyon Way, Frimley Road, Frimley, Surrey, GU16 7ER

The role will require working across the practices within the Federation / PCN footprint and the care home settings across the footprint, as well as a requirement to work with colleagues across the Surrey Heath Integrated Care Team (ICT)


Hours per week:37.5 hours per week (Full time). Part time hours considered


Salary:
Agenda for Change Band 6 Equivalent, depending on experience and qualifications (pro-rata for part time)


Annual Leave: 27 days (based on full time hours, pro-rated for part time) plus Bank Holiday entitlement

Our Service:

Surrey Heath Community Providers offer a 365-day, 52-week service delivery and as part of that, we offer a seven days' a week, 8:00am to 8:00pm service.

So as part of your normal working hours, you may be required to work evenings or weekends, to provide a seven-day service, as per the needs of the business.

We began in 2016 and now employ around 200 members of staff across our clinical and corporate support teams. Since 2019 we have hosted the Primary Care Network in Surrey Heath.

JOB PURPOSE

  • To work as part of a positive, proactive and collaborative patient/resident/clientfacing multiprofessional primary care clinical team supporting general practices in Surrey Heath. The role is a key member of both the Surrey Heath Federation/PCN clinical Care Homes Team and the wider Surrey Heath general practice team.
  • Working with the Advanced Specialist Dietitian to support the Practices by providing expert advice, guidance and recommendations to care homes, GPs and other healthcare professionals, and identifying areas for improvement and initiating and managing change where appropriate. Taking referrals from Practices to support the delivery of high quality nutrition and hydration care, recommending and facilitating sustained changes in prescribing practice to ensure high quality, safe, evidencebased and costeffective nutrition and hydration care and support.
  • Working as part of the PCN dietetic and wider Care Homes team to support the Primary Care Network (PCN) implement the nutrition and hydration requirements of the new Network Contract Direct Enhanced Service and other PCN Care Homes responsibilities, including nursing and residential home settings for adults with social, physical and mental health needs and people with learning disabilities. The service is designed to develop and embed positive, proactive care for people in care home settings.

CORE CLINICAL DUTIES AND RESPONSIBILITIES

  • Provide specialist nutritional assessment, management and treatment of patients within the area of adult nutrition support in care homes (excluding patients on home enteral feeding).
  • Provide specialist nutrition and diet advice to patients, their carers, and healthcare professionals through treatment, education plans, and prescriptions.
  • Educate patients with dietrelated disorders on how they can improve their health and prevent disease by adopting healthier eating and drinking habits.
  • Negotiate and agree dietary changes/treatment options using appropriate counselling, motivational and behaviour change techniques and skill. This may also involve patients' carers, other family members or members of the MDT where appropriate.
