An exciting opportunity has arisen for an enthusiastic, creative and experienced health practitioner to join the Homefirst Team in Kerrier.
This position is open to a Physiotherapist; Occupational Therapist or registered RGN who has experience of working in the community and wishes to develop his/her rehabilitation skills whilst being part of a dynamic, progressive, integrated multi-agency, multi-disciplinary team.
The successful applicants will be based in Camborne Health Office.
The Homefirst Team is part of the integrated community team in Kerrier which includes registered and non-registered nursing, therapy and administrative staff who work closely with partnership organisations to support the intermediate health needs of our local population.
You will be responsible for assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identify problems and needs of patient and set realistic goals.
You will need to have excellent organisational skills combined with a wide knowledge base and experience of working in older person's services.
