Job overview:


The post is to support training and education within our Community Diabetes Services, the post holder will also work clinically with the Specialist Diabetes Team, Primary Care Partners and the Clinical Practice Educators.


There will be clinical element within the post, to support Primary Care in achieving the Diabetes 8 Care Process for patients in Oldham.

This post is open to Nurse Associates or Assistant Practitioners to apply.


The work will take place in a variety of community settings including patients homes, nursing/residential homes and intermediate care units.


The post holder will often work in the community as a lone worker, in line with organisational lone worker policy.


This post is to be offered as a 12 month fixed term or 12 month secondment for internal members of staff.

Main duties of the job:

An exciting opportunity has arisen for a Nursing Associate or Assistant Practitioner to support training and education in the community Diabetes Team

The post holder will support service improvements and education in practice within this speciality in Oldham by:

  • Keeping the people who using our services as safe as possible through the use of sound clinical skills and effective risk assessments.
  • Ensuring the best possible clinical outcomes by using uptodate skills and adhering to evidencebased policies and procedures.
  • Ensuring the people using our services have a good experience by respecting, empowering, and working in partnership with people throughout the care planning process.


The main role of the education associate is to work within a community-based Specialist Diabetes Team in the implementation, delivery and evaluation of defined education programmes.

Defined clinical interventions to support Primary Care in improving health outcomes in the diabetes population.


The Nurse Associate will take responsibility for their own workload within a broad framework laid out by a qualified Specialist Diabetes Nurse.


The post holder will be responsible for delivering defined training and education without direct supervision but will have immediate access to a Specialist Nurse for advice and direction as required.

  • Ability to develop effective and appropriate relationships with people, their families, carers and colleagues
  • Ability to support, supervise, assess and act as a role model to nursing associate students, other learners and health care support workers as required within the clinical setting
  • Maintain high standards of quality in corporate and clinical record keeping ensuring information is always recorded accurately, appropriately and kept up to date
