Calderdale Council are looking for Agency Social Workers to join their
Discharge to Assess Team.

Job Criteria:

Experienced SWs required for immediate start, working within the Post Discharge Assessment Team (PDAT)


You will be responsible for carrying out holistic assessments of people in their own homes or temporary placements, this may include completing Care Act Assessments, MCAs, Bis, supporting with housing issues, giving information and advice re financial costs etc.

We work with Calderdale adults over the age of 18yrs but predominantly older people.

Currently we have people placed in carehomes in Calderdale, Bradford, Kirklees and East Lancs so you will need a full driving licence and access to a vehicle on a daily basis.

PDAT is a fast paced team with a quick turnover of cases.

Most work is carried out remotely with an expectation that all assessments and reviews are carried out face to face with the client.


Paperwork can be completed at home but there is a council wide expectation that all staff are office based at least one day per week.

Whilst undergoing training it is likely that you will be office based more often.
