Mental Health Practitioner, Band hours a week, fixed term for 12 months)


We are looking for a compassionate, skilled Mental Health Clinician (2-3 days per week) who would like to focus on triage, assessment and MDT decision-making for our service.

Secondment can be considered with your manager's approval.


This post works into a newly formed multi-disciplinary Triage Team as a pilot project within the Specialist Weight Management Service.

This pilot will also contain a G.P, dietetic assistant, and assistant psychologist, with dedicated support from management and an admin post.


The role will involve screening referrals for risk, suitability for the service, liaising with services and patients, collating and interpreting patient data including questionnaires, and providing clear plans and recommendations to best meet referred patients' mental health needs.

Office hours only, no out-of-hours required

The new Triage Team is a 12-month ICB-funded pilot project aimed at:

  • Providing early vetting and screening of referrals and appropriate gathering of further information, to ensure quality and timely referrals onwards, where indicated
  • Earlier provision of personalised recommendations to optimise comorbidity management, improving healthcare outcomes and promoting best care for people living with weight
  • Improving referral quality and treatability for those added to the waiting list
  • Supporting faster transition to and completion of clinical assessment, contributing to increased throughput to treatment
  • Support in managing and reducing current backlog, reducing harm to those on the waiting list by earlier identification of risk issues or unmanaged comorbidity
  • Collection of relevant information for any gaps in provision, particularly, those linked to health inequalities.


The Mental Health Clinician will lead on the vetting, screening, advice, guidance, planning regarding referred patients' mental health needs and form a core part of MDT decision-making.


The Specialist Weight Management Service (SWMS) is an MDT formed of psychologists, dietitians, nursing, physician time and an admin team.

It is based at the Gloucestershire Royal Hospital in Gloucester.

The team are passionate and motivated in their work with an ethos of keeping patients at the heart of what they do.


  • To play a key role in the mental health aspects of vetting and assessment of referrals using patient questionnaire data, medical data, structured tools and where appropriate direct patient contact via telephone
  • Develop clear outcomes and plan following triage
  • To be part of MDT decisionmaking about the suitability of patients referred and any referrals on to other mental health services required
  • Advocacy for patient group within primary and secondary care
  • Support more efficient decision making and faster throughput to treatment via robust Triage and screening
  • Screening for risk and raising potential risk with colleagues, overseeing relevant management and making onward referral where indicated
  • Liaison with external colleagues e.g. mental health teams, primary care
  • To assist in the undertaking of any research projects, audits and outcome data for the project
  • To provide effective advice and guidance to the MDT, wider professionals and where appropriate, the referred patients
  • Participate in the education and oversight of junior staff
