The shift pattern for this post is Monday - Sunday

An exciting opportunity has arisen for a Band 4 Rehabilitation Support Worker to join a well-established Community Rehabilitation Team.

The Rehabilitation service responds to referrals from a range of areas which include; Acute hospitals, Emergency departments, Community settings, and Specialist Services

Within the Rehabilitation Service the post holder will work with our OT, Physio, nursing, dietetic and SLT staff to provide ongoing rehabilitation input, progression of rehabilitation interventions and complete treatment to the point of discharging from the rehabilitation service with a defined group of patients.

The post holder will work as an effective and efficient partner in the interdisciplinary team, incorporating extended role competencies.

The post holder will primarily have a clinical role, but will participate in health promotion, audit projects, student and staff induction and education

A current full driving licence is required
