An exciting opportunity has arisen for a 12 month secondment/fixed term for a Personalised Care Lead and Cancer Improvement Facilitator, based at The Royal Shrewsbury Hospital (travelling to Princess Royal Hospital, Telford, as required).


This role will support operational and clinical teams with the implementation of the core elements within the Personalised Care agenda as per the NHS Long Term plan, using the comprehensive model of care as a driving force.

In order to do this, the post holder will support clinical work streams and other key stakeholders to identify opportunities for personalised care transformation, creating culture change across the organisation through promoting key components of personalised care i.e.

supported self-management, social prescribing, personalised care planning, shared decision-making, and patient choice.

In order to implement and support the change process, they must be able to assess and develop training needs across the Trust with a vision of implementing the core elements of the Personalised Care agenda.


Ideally, the post holder will have experience with working with partners across the local health system to develop the personalised care offer in the local health economy.

The post holder must be able to develop good working relationships with Primary Care Social Prescribers/Provider organisations to ensure strong inter-organisational working for the benefit of the patients.


The post holder will empower patients to live well through better, meaningful conversations, self-management education, care coordination and facilitating peer support.

Influence and support staff to embed personalised care planning and shared decision making in practice.

Contribute to the development of the personalised care strategy for the local area and Trust.

Facilitate the implementation of transformation and pathway re-design identified through improvement activity and feedback.

Work with collaboratives to ensure full implementation of the personalised care agenda. Prepare and facilitate training sessions and other workshops as required around personalised care.

Work to tight deadlines and facilitate delivery of multiple projects to agreed quality standards/outcomes.

Gather data and analyse benchmarking information for personalised care trends and identify opportunities for transformation.

Participate in the development of mechanisms to monitor and evaluate transformation initiatives and measure sustained change.

Collaborate with service leads regarding current practice and processes, which can involve positive changes and challenges regarding service change.

Identify opportunities and work with key stakeholders to implement new initiatives to support those affected by cancer.

The Cancer Services Department is seeking to recruit a Personalised Care Lead and Cancer Improvement Facilitator who exemplifies the values to join our team, based at The Royal Shrewsbury Hospital.

As a department, we work closely with the clinical teams throughout the Trust. We promote a friendly, approachable and supportive team atmosphere to deliver a high standard service.


Although this job role is not Nursing and Midwifery registered, the hours accrued will count towards practice hours required for revalidation.
