Description

To provide strategic leadership to the Virtual Hospital in all current pathways,, together with the future development of the model, together with additional initiatives as agreed by SWHHCP The post holder will demonstrate understanding of the national strategic direction of the VH model,, ensuring th VH performance and delivery operates in line with relevant guidance, policies, and procedures and maintaining the SWHHCP VH programs position nationally as an exemplar for VH care To work collaboratively with partners in the SWHHCP to ensure effective and efficient delivery of all pathways within the healthcare system To be responsible for the delivery of monthly performance reporting for all pathways and ensure standard operating models for performance management are deployed.
