Description

You will be a senior member of staff working as a triage practitioner within the integrated 0 to 19 Torbay Partnership.

Providing the expertise, knowledge, resource and support to the team managing enquires from pregnant women, families, carers, children and young people.

Confidence to provided a comprehensive triage service for the public and professionals.


You will access and update the Health record to ensure continuity of care and to make appropriate recommendations and interventions.

The service includes the identification of vulnerable children, including those whose welfare and safety may be at risk, working within safeguarding procedures and in collaboration with other agencies.


You will have the opportunity as part of a team to shape the ongoing development of the role and to interface with a wide range of internal and external partners in providing care to children and young people in Torbay.


  • The ability to communicate highly sensitive, complex or contentious information in a professional manner.
  • Link with surgeries supporting the PHE Hepatitis B pathway, blood spots results in line with the PHE pathway, triage questionnaires for children and young people over 5 years.
  • Have knowledge of our Partner agencies services and services that may be available within the wider community.
  • Be up to date awareness of current local and national policies.
  • Be able to use and maintain IT data bases and have a basic understanding of spreadsheets.
  • Develop and shape this post and will work with the management team to realise the potential of this post.
  • Provide placement and mentorship for students.
