A position has arisen for the role of Patient Pathway Co-ordinator within the Cardiology Team at Kings Mill Hospital.


We are looking for an enthusiastic and motivated team player with empathy, compassion, an excellent communicator & good problem solving skills.

You will require high levels of tact and diplomacy with the ability to prioritise your own workload effectively to enable day to day management.


You will be working within a team of 4 other Patient Pathway Coordinators, 2 Records Assistants and 5 Clinical Typists.


The role of a Patient Pathway Coordinator is similar to a Medical Secretary role in which you will provide high quality administrative support to the team of Cardiology and provide excellent Patient care to all of those under our service.


On a day to day basis this would involve:

  • General admin, receiving and managing incoming post and referrals and distributing out to the Cardiology team.
  • Managing the Patient tracking list to ensure all patients are treated within a timely manner and adhering to National operating standards.
  • Booking patient appointments.
  • Audio typing.
  • Managing Clinician diaries and annual leave, cancelling clinics when needed.

To be the main point of reference for the Cardiology team to ensure administrative support throughout the patient's journey at the hospital working as a team with Patient Pathway Co-ordinator colleagues to ensure appropriate cover.

Ensure that all appointments across the patient pathway from referral to discharge are booked in line with targets/standards. Undertake validation of the RTT Patient Tracking List.

Provide a patient centred and timely response to all telephone enquiries from patients.

Ensure all mail is sorted and correspondence dealt with in a timely fashion.


Investigate and take the appropriate action where pathways are incomplete to ensure that patients are receiving timely treatment and Trust information is robust following agreed on escalation procedures.


Typing of patients clinical letters and liaising with GP's and community nurses to ensure patients prescriptions are actioned and tests are completed.


Liaise with the Clinical Typist to direct and ensure all Outpatient Clinic letters, operation notes and ward discharge letters are typed in accordance with typing turnaround standards.


Confirm in writing, details of the agreed appointment dates, thus ensuring patients have all the information required for their outpatient visit.

Change and cancel Clinic appointments as appropriate and in accordance with the Trust Access Policy.


Liaise with Clinic Prep Clerk / Records Assistant as appropriate to ensure all case notes are available and fully prepared for the patient's attendance at Clinic.

Ensure all Clinics are fully reconciled in conjunction with the Clinic receptionists.

All outcomes should be actioned within 24 hours in line with Trust SOP and protocols.
