Primary and Community Services are at the heart of the model and central to developing a new relationship with patients investing and strengthening community and social care services to support and treat patients in their homes and communities providing quality outcomes.

In order to deliver the strategy we are offering exciting opportunities working closely across the health board and in partnership with key stakeholders.

We are looking for an inspiring nurse who is enthusiastic and self-motivated individual to work within the District Nursing service to deliver a broad range of clinical interventions and share their expertise.

The individual will be required to undertake managerial responsibilities as well as delivering teaching to less experienced staff.

If you have the Community Specialist Practitioner Qualification and significant experience of community nursing you are invited to apply.
