Description

We are looking to recruit an experienced, motivated individual to join our complex discharge team.

You will be predominantly based at the St Mary's Hospital site but will be expected to work across site as required.

The post holder works alongside the Specialist Discharge Coordinators and Specialist Discharge Nurses to support the Trusts agenda to minimise delayed discharges.

The role will involve responsibility for coordinating complex patient discharges. This will be achieved through attending ward multidisciplinary meetings and daily ward visits and close working with nursing staff and the multidisciplinary team
