£24,496 per annum, pro rata for part time

INT Care Co-ordinator - Stowmarket - Part Time 20.5 hrs per week

I NT Care Co-ordinator - Eye - Part Time 20.5 hrs per week

These positions are joint positions, if you are currently on NHS Terms and Conditions you will be able to stay on these terms.

If you are a current member of Suffolk County Council or an external applicant, you will be employed under Suffolk County Council Terms and Conditions.


Co-ordinating the care to keep people well at home and maximising their independence through transforming the way we work, building relationships, reducing duplication, unnecessary admissions to hospital and delivering services closer to home.


Liaising with individuals and families to ensure they have access to the best possible support & services for their needs.


Working in a friendly and supportive environment, alongside professionals who are committed to developing the service, such as social care teams, community health teams, statutory services & voluntary organisations, NHS colleagues, and other organisations within their designated locality.
