Description

Supporting the Consultants in leading and supervising the team in the day to day management of patients following a comprehensive geriatric assessment approach.

Team members should attend a daily huddle or SAFER board round to ensure prioritization and efficient use of time. Working closely with the wider multidisciplinary team to ensure patients are managed effectively and efficiently Proactively liaising with patients families / next of kin (with patients permission where they have capacity or otherwise in their best interests) to ensure an understanding of frailty and management plan Supporting junior doctors in their training and completing assessments All members of the team are responsible for ensuring patient flow and effective bed usage by prioritizing discharge planning / completion of Electronic discharge letters Supporting the managers with rotas / leave Engaging with quality improvement work and data capture
