Our
Team Leaders here at Change Grow Live play an integral role in ensuring an effective service provision for our service users by creating a focussed culture, engaging staff to provide an innovative and continually improving service and provide leadership, support, and development to a wide range of staff.
We have a new and exciting opportunity for a Partnership
Team
Leader to join our Eaing service as part of our multi-disciplinary team.
You will work flexibly and creatively to establish what is needed to have the most positive impact for all who are referred to our service.

£4,072.06 Inner London Weighting
Hours: 36.5 per week
Salary:
£34, £36,635.38 dependent on experience
Responsibilities:
About the role:
- Lead and coordinate data analysis to feed into ICS and CDP developments
- Map, monitor and understand the local Integrated Care System (ICS) and related
- Take on project management of any partnership developments including
- Develop relationships with CDP and subgroups, Primary Care Networks including
Teams, Acute trusts and VCSE alliance
- Identify opportunities and embed partnership development events across the
- Be part of the service leadership team contributing to priority setting, attending
- Communicate with national organisational leads regarding ICS and CDP
About you:
- Educated to degree level or equivalent relevant experience.
- Knowledge and experience of developing and leading health and social care services and delivering services which evidence high performance and quality standards.
- Knowledge of current legislation surrounding children, young people, and safeguarding practice
- Able to lead, inspire and develop a team.
