Overview:


Our
Team Leaders here at Change Grow Live play an integral role in ensuring an effective service provision for our service users by creating a focussed culture, engaging staff to provide an innovative and continually improving service and provide leadership, support, and development to a wide range of staff.


We have a new and exciting opportunity for a Partnership
Team
Leader to join our Eaing service as part of our multi-disciplinary team.

You will work flexibly and creatively to establish what is needed to have the most positive impact for all who are referred to our service.

Allowances:
£4,072.06 Inner London Weighting


Hours: 36.5 per week


Salary:
£34, £36,635.38 dependent on experience


Responsibilities:


About the role:


  • Lead and coordinate data analysis to feed into ICS and CDP developments
  • Map, monitor and understand the local Integrated Care System (ICS) and related
activity

  • Take on project management of any partnership developments including
partnership projects funded through additional funding grants

  • Develop relationships with CDP and subgroups, Primary Care Networks including
Clinical Directors, local place-based partnerships, Mental Health Transformation
Teams, Acute trusts and VCSE alliance

  • Identify opportunities and embed partnership development events across the
service to support specific learning/activity that will contribute to reducing drug and alcohol related harms and address local health inequalities

  • Be part of the service leadership team contributing to priority setting, attending
relevant internal and external meetings (monthly/bi-monthly/quarterly)

  • Communicate with national organisational leads regarding ICS and CDP
developments and cascade to service


About you:


  • Educated to degree level or equivalent relevant experience.
  • Knowledge and experience of developing and leading health and social care services and delivering services which evidence high performance and quality standards.
  • Knowledge of current legislation surrounding children, young people, and safeguarding practice
  • Able to lead, inspire and develop a team.
