  • The role
  • Herefordshire Council Community Wellbeing Directorate; Care Act Assessment Team is looking for two fulltime Social Care Assessors to join us in providing quality services for vulnerable adults in Hereford.
  • The focus of this team is carrying out short term assessment & support planning with service users throughout Herefordshire who have recently left hospital and require ongoing services. This is an exciting opportunity to join our team and we are looking for enthusiastic staff who enjoy fast paced work to set timescales and are keen to support service users.
  • The successful Social Care Assessors will have experience of working with vulnerable adults with a range of physical disabilities, learning disabilities, mental health needs, hospital discharges & community services. Experience of assessment and support planning skills would be an advantage, as well as knowledge of the Care Act An excellent communicator with good organisational and IT skills, you will be looking for to further develop in a career that offers great progression and makes a difference to adult's wellbeing.
  • You will receive regular supervision, working in a supportive environment with other social care staff and have a range of benefits to ensure a good worklife balance while also being able to conduct the full requirements of the role.
