Ealing Intermediate Care service has an exciting role within the local management team for an enthusiastic and motivated senior Nurse or Therapist with leadership experience and experience of working within a multiple disciplinary team at a senior level to provide daily operational leadership in line with the agreed service.


Your main role will be to provide sound clinical and operational support with the oversight of the Matron or Admission Avoidance.


  • Day to day line management of RR team
  • Ensure operational delivery of RR pathway
  • Work seamlessly with peers in other community teams
  • Act as point of reference for team
  • Act as point of contact for internal and external staff
  • Ensure staff are compliant with training, appraisals and supervision
  • Ensure WLT policies and processes are adhered to

  • To provide clinically excellent leadership across IC and develop peer network across ECP, WLT and CNWL.
  • To contribute to development of the admission avoidance pathways across IC, ECP and NWL.
  • To ensure the successful implementation and roll out of the admission avoidance pathway and the Quality Improvement Programme.
  • To work closely with the MDT in providing seamless care across departments and professions
  • To be responsible and accountable for patient flow and service delivery across the admission avoidance pathway
  • To develop teaching and career pathways across IC and ECP
  • Hold and manage admission avoidance therapy budget

Key Result Areas & Performance:

  • Responsible for day to day admission avoidance services provision across inpatient and community teams including work allocation, performance/attendance issues, participating in recruitment, appraisal and personal development planning.
  • Develop CGA and frailty model of care across IC with the Clinical Lead and therapy lead.
  • Responsible for supervision of staff, including work allocation and checking.
  • Plans and leads on quality improvement work across IC and ECP with peer network
  • In collaboration with therapy lead to ensure D2A P1 is fully embedded within IC and ensure effective flow.
  • Aligns teams with the wider professional groups within WLT and CNWL.
  • Ensuring teams are paper light and adhere to IG guidance
  • Embed and maintain high standards of interventions and benchmark against national, local and inservice standards.
  • Manages and develops practice development role within IC
  • Works seamlessly with the locality service team to embed planned and unplanned pathways across ECP.
