Neurodevelopmental Support Coordinator


Location:
Central Derbyshire with main base in Chesterfield


Rate of Pay:
£23,961.79 full time equivalent


Hours:30 hours per week (£19,428 per annum)

(Fixed Term Contract for 12 months)

Overall Purpose of Post


To establish and develop Community Hubs that offer provision and support for young people with Neuro Development needs and their families in Chesterfield, Ripley and the surrounding areas of mid Derbyshire.


Responsibilities and Duties


To be responsible for ensuring that key services such as 1:1 information and advice, needs and profiles, referral plan, client satisfaction and indicator capturing and reporting for Community Hubs, offering drop-in sessions, advice and ongoing support for young people with neurological conditions are effective.


To promote the hubs services to young people and their families through networking with identified stakeholders, community pathways and social media.


To coordinate and support a wide range of exciting projects offering support to those with neurological conditions including but not limited to, a community café, parent/ carer support groups, youth activity groups and drop-in centres.


To ensure the hubs hold relevant, up to date information for attendees and information to facilitate signposting to other services where necessary and to grow the list of organisations providing services and identify and try to fill gaps in provision.


To ensure facilities, processes and equipment are set up for opening and maintained and fit for purpose through regular health and safety checks and that the venues are suitable for neurodiverse clients.


To ensure policies and procedures and those specific to the hubs are followed and to be involved in regular policy reviews and risk assessments.


To support start up and ongoing training, identify further training to support hub teams, attend training sessions relevant to the post and to help organise training sessions when and where appropriate with the relevant agencies.

To offer support to the team working in the Community Hub café and other staff through regular team meetings.

To liaise closely with professionals working with identified families and individuals.

To meet with statutory and voluntary partners / stakeholders to promote joint working with Community Hubs.


To promote the Community Hubs and to liaise with other organisations including those in GP's surgeries, Children's Centres and Crisis Cafes.

To be aware of local and national initiatives regarding issues relating to children and young people with neurological conditions.

To gather data and produce reports evaluating the success of the Community Hubs.

To produce monthly reports for the Project Management Team and Commissioners.


To comply with Health & Safety requirements and specifically to take reasonable care of themselves and other persons who may be affected by their acts or omissions at work (Health & Safety Act 1974) and other relevant employment legislation and Fairplay policies.


To carry out any other duties appropriate to the post, as designated by the Chief Executive and Hub Board of Trustees.


Person Specification Neurodevelopmental Support Coordinator

Qualification
NVQ Level 3 or above in a related field


Essential Criteria
Experience of working with families with complex needs in partnership with other statutory / voluntary agencies

Experience of working in, and contributing to multi-disciplinary teams

Experience of line management responsibility

Experience of working with young people who have neurological conditions

Knowledge of Safeguarding Children guidelines

Knowledge of disability issues

Ability to work on own initiative or as part of a team

Ability to produce reports and risk assessments using IT skills

Ability to maintain confidentiality

Ability to demonstrate sensitivity and a high degree of discretion

Ability to develop and maintain relationships

Ability to travel


Desirable Criteria
Experience of being part of a management team

Experience of facilitating training


Other Requirements
Ability to work flexibly including some evenings, weekends and school holidays
