Description
The post holder is a highly specialised nurse working as a member of the paediatric cardiac nursing team.
The team functions as part of the wider multidisciplinary team to provide advice, information and support to children with congenital heart disease and their families.
The post holder will act independently/ autonomously within cardiac team to manage caseload being guided by best practice and trust policies and guidelines.

We also aim that all members of the Paediatric team will work in an environment which provides support and mutual respect for each other.
- Work effectively within the paediatric cardiac specialist nursing team ensuring a very high standard of care throughout the patient's journey from the earliest point at which concern for the child is raised to the preparation and transition to adult care.
- Have ongoing responsibility for own caseload building the relationship with each family but delegating care and referring on as appropriate (current individual caseload
- Using expert judgement and analytical, problem solving skills (evidence based) in the surveillance and recognition of changes in patient's condition referring changes to the clinicians as necessary.
- Give support that is appropriately planned to meet each individual's needs empowering them to manage their care, to recognise any deterioration or acute episodes requiring immediate specialist attention and how to access the necessary support. This must be a dynamic process recognising the growth and development of the child
