Care Coordinator
£22,000 - £24,00 per annum (plus travel)

Key responsibilities of our Care Coordinator:

  • Engage with clients, their families and carers in the planning, delivery and development of person centred care.
  • Review clients care plan and risk assessments ,updating and inputting data monthly
  • Record evidence of positive and negative outcomes from the delivery of care and inputting data monthly
  • Make recommendations and changes to the care plan whilst involving clients
  • Monthly checks of clients hoist, slings and any other equipment used as per care plan
  • Maintain relevant records and reports accurately
  • Respect and maintain confidentiality at all times in accordance with Percurra policy and procedures Liaise with and providing support to care assistants working within the team of the care coordinator
  • Inform the team of any changes as soon as they occur
  • Supervision of care assistants allocated to the coordinators team, and records kept of the outcome
  • Liaise with clients, families, carers about changes to times and details of care, on a weekly basis, reporting any changes to customer services.
  • Cover on call as per roster
  • Monthly meetings with care manager
  • Immediately report to the care manager any shortcomings in health and safety procedures and practices
  • Report any accidents or dangerous incidents to the care manager as soon as possible and submit a completed accident/incident form
  • Act In A Supportive, Informative And Sensitive Manner Towards Clients, Families And Carers At All Times
  • Comply With All Policies, Procedures And Practices And Keep Up To Date With Any Changes To These
  • Establish And Maintain Effective Working Relationships
  • Perform Other Related Duties As Required
