Job overview:
This is an exciting opportunity for an enthusiastic and highly motivated individual to join the Dementia Older Peoples Mental Health (DOPMH) service in the North and East Cornwall.
The successful applicant will have the choice to be based at either Banham House, Bodmin, working with the Camel team DOPMH team, or at Elfordliegh Resource centre, Launceston, working with the Valency DOPMH team.
The DOPMH service strives to deliver high quality and effective services to people experiencing a dementia illness and a functional mental health illness age over 75.
We are looking for someone, who has a core profession in mental health nursing, social work or occupational therapy to join our DOPMH service line.
We want people who have experience or who are looking for an experience in working independently in a mental health setting.
As a team member you must have a positive outlook and sense of humour to accept the challenges of working in a busy locality that extends to seaside towns and rural locations.
You will need to have knowledge of general mental health problems, the impact this may have on service users and their treatment.
You will need excellent communication skills and place value on team and inter-agency working.We need a person who can demonstrate passion and commitment to mental health issues have an approach to excellent client centred care along with working flexibly as part of a team.
Main duties of the job:
- To be responsible for the assessment of care needs and clinical management of a defined caseload within the Cornwall Partnership NHS Foundation Trust (CFT) Mental Health Service.
- To carry continuing responsibility for the development, implementation and evaluation of programs of care, utilising the care coordination system.
- To develop the interface between primary and secondary care services so the patients/carers
